Children & Adolescents
We offer Children’s Case Management Services to assist youth and families with mental health issues, developmental disorders, and chronic medical conditions. We recognize that families can face unique challenges and assist with developing an Individual Service Plan (ISP) detailing the strengths and needs of the child and the services necessary to address these needs. In addition, our trained Case Managers will work with the family to assist with the child’s coordination and advocacy of services.
Our Services
We are licensed by the Department of Health and Human Services and serve eligible youth from birth to age 20. We assist families in building independence, enhancing functioning, improving outcomes at home, school, and within the community, and empowering youth and families.
