The Transition House
Mental Health Program
The Transition House Mental Health Program (THP) is a residential mental health facility located in Dixfield, Maine, serving adolescent youth ages 13-18 who have a mental health diagnosis and can benefit from residential treatment. The facility provides services for male adolescents from throughout the State of Maine. Services include housing, education, recreation, individual/family counseling, and job placement opportunities. THP is licensed by the Department of Health and Human Services as a residential mental health facility. It is the mission of the Transition House Mental Health Program to provide services, supports, and opportunities to guide youth in a healthy lifestyle as they gain independence in the community. Client referrals into the program come from correctional facilities, other primary treatment facilities, individual private therapists, and the Department of Health and Human Services. Our treatment approach at the Transition House is based on trauma-focused cognitive behavioral under the direction of the program Clinician and the Director of Programs.
The goals of the Transition House Mental Health Program are to:
- Provide "after-care" services in a safe and supportive residential environment;
- Safely integrate youth into their school and community at large while affording great opportunities for growth and independence;
- Encourage responsibility for behaviors;
- Make positive changes in contributing factors to past behavior/social problems;
- Assist in the development of empathy;
- Teach healthy boundaries, relationships and sexuality;
- Pro-social interactions with others;
- Support positive family interactions;
- Practice new skills within the community, home and school;
- Teach activities of daily living and independent living skills.
Education:
All youth who reside at THP are enrolled in the public school system of MSAD #21, Adult Education, NOVA-vocational or Pre-vocational programs, according to age appropriateness and ability level. Support services such as Pupil Evaluation Team (PET) meetings, remedial materials and tutoring services from the community may be provided to support the resident in achieving his educational goals. MSAD #21 also provides summer school opportunities. Youth who have received a General Educational Diploma will participate in a career development program. The program also provides daily study time when school is in session.
Recreation
Implemented as part of the THP Program is a wide range of recreational
and social activities which reinforces positive interaction among
both staff and residents. Activities include:
camping, hiking, golf, tennis, ice fishing, downhill skiing and
many other individual and group activities.
Supporting these residents are a qualified
staff of: a program director, case manager, clinician, eight full-time youth
workers and on-call relief staff. The
staff are all trained in dealing with the behavioral dynamics of
troubled individuals and have experience and/or education in the
social services field. Youth workers also provide 24 hour awake
supervision of the residents. The youth
workers primary responsibilities are to model appropriate behavior,
support the residents with their individual service plans and supervise
residents at all times.
